AGREEMENT FORM

This agreement form will be emailed to Randall Skow, who handles the processing of agreement forms, when you click on the 'Submit this Agreement Form" button. You will also see your details in a response web page that only you will see. Print out that web page for your records.

  • This is your first version of the agreement.

  • Randall will acknowledge your sending of this agreement form in an email

  • If you do not receive an email from Randall acknowledging your submitting of this agreement, please be sure to ask him about that since the form may have been lost in cyberspace (almost never happens).

    If there are any changes that Randall has made,
  • Randall will send you a message with the changes.

  • You should acknowledge and agree with any changes by replying to that email.

  • Randall will again email you to verify that you and he have agreed upon any changes.

  • This process will assure both you and Randall that the agreement details are all understood by everyone.

    By filling out this form, you are obligating yourself to these details. By acknowledging your form submission in a confirming email to you, Randall Skow is obligating himself to these details and dates, times and locations.

    Base Rate

    The base rate includes a maximum of 2 hours, 2 loudspeakers on tripod stands, one wireless lavaliere or headset microphone. Four Microphones on cables will be available if needed. The site provides electrical power. The Base Rate is a charge for actual performance. No additional charges for setup or packup. The Base Rate is $250 for any type of event.

    Additional Expenses That May Be Required

    [$100] 4 hours of Battery Power, if site does not have adequate power.
    [$100] Audio Recording (Mono) of event by Sony MiniDisc (~160 minutes max) and conversion to CD as .wav or .mp3
    Sales tax for Audio Recordings
    2 Additional Speakers on Stands
    Round Trip Mileage [@ $0.50/mile + fuel surcharge, first 20 miles free]

    I understand that Randall Skow will arrive at my event location at a time that is approximately one hour before the event is scheduled to begin, unless we have arranged with Randall Skow in writing - by email - to be there at some other time.


    A deposit has been given to Randall Skow (the deposit applies towards the total fee).


    I agree to forfeit the deposit if I do not use Randall Skow's services due to any changes that I make in the schedule, due to cancellation of the event, or if I decide to have another technician.


    I agree that I will NOT have to pay the balance due if the event is cancelled or if Randall Skow's services are not used for the event. I agree to inform Randall Skow at least two weeks prior to the event date if the event has been cancelled.


    I understand the following limitations and realize that the services for this event would not be provided under these adverse conditions:

    Equipment limitations for outdoor use
  • Minimum operating temperature is 45F.
  • Maximum operating temperature is 100F (Inverter will define upper limits)
  • Cannot perform amplification services in an unprotected outdoor environment if it’s raining or if the ground is excessively wet due to rain.
